BC Bibliography: The University of British Columbia Library maintains the Bibliography of Theses on British Columbia History and Related Subjects. This important online reference source is available on the Internet through the UBC Library online catalogue. The Bibliography is used by students and researchers at UBC and other libraries and research institutions in British Columbia, Canada and other countries. If your thesis is relevant to British Columbia (e.g. history, geography, literature, science, special topics, etc.) you are asked to complete this form, and submit it with your final thesis.
Thesis/Dissertation Submission Cover Sheet: This form must be submitted with all Master Thesis and Doctoral Dissertations. When you check the box indicating that you are submitting your thesis or dissertaion in accorddance witht the formatting requirements issued by the Faculty of Graduate Studies you are confirming you have complied with the copyright requirements.

Library and Archives Canada Non-Exclusive License: For a doctoral dissertation to be microfilmed by Library and Archives Canada, you must submit a signed Non-Exclusive License form with your dissertation. This signed agreement allows Library and Archives Canada to list your thesis in the national bibliography Canadiana, and in their online bibliographic catalogue, AMICUS Web. This form also authorizes Library and Archives Canada to reproduce, loan, distribute, or sell copies in microform, paper, or electronic formats on request through University Microfilms International (UMI), the microfilm service provider for Library and Archives Canada. You can find more information on the Theses Canada web site. Information provided on the Library and Archives Canada form should be typed, but if handwritten, black ink should be used. The "Date Degree Awarded" is the month and year of your graduation ceremony.
Thesis Licence Agreement: This form grants a licence to the UBC Library to make your thesis available for circulation. If material in your thesis is copyrighted to a third party, the third party must also agree to the terms of this agreement. You must fill out and sign one copy of this form and submit it with your final thesis.
Withhold Thesis from the Public Domain: Under certain circumstances, theses may be withheld from the public domain for a limited period of time, at the request of the student and the supervisor. This will typically be for 6 months, extendable to a maximum of 12 months under special circumstances, and with substantive rationale. You might wish to request that your thesis be withheld if any of the following circumstances applies:
