Signature Form The Internal UBC Okanagan Signature Page must be completed and submitted to the Office of Research Services with all external grant applications Applications cannot be accepted without a completed signature page. *Note: If you require additional space for co-applicant signatures (i.e. there is more than one co-pi or co-applicant), please submit additional signature forms to accommodate signatures

Grant Information Form (GIF): To set up a Research Account, the Office of Research Services requires a copy of the application for funding. In the event that a formal application is not provided or required by the funding agency, the researcher must complete the Grant Information Form. For additional information on application procedures, click here.

Request for Issue of a Subgrant: This form is to be used for Inter-University Transfers. 

Study Leave and Salary Research Grant Forms: These forms are used for applying for a Study Leave or Salary Research Grant.
