You have already established eligibility for academic accommodations with the DRC. Here is the form to request your accommodations at the beginning of each term.
Alternate Format Request Form - [57KB]
You require alternate format reading material. Complete this form for EACH item you wish to be converted to alternate format. Bring completed forms to the DRC.
Exam Booking Form - [101KB]
You need to write an exam through the DRC. Complete this form with your professor and bring it to the DRC at least one week before the date of your exam.
Intake Form - [37KB]
To initiate registration with the Disability Resource Centre complete this form and bring to the DRC - UNC215 or send to jess.roebuck@ubc.ca.
Permanent Disability Programs Application - [246KB]
You want to apply for Canada Student Grant, CSG Services and Equipment or need a form for your doctor to fill out to verify your disability. Complete this form and bring it to the DRC.
